On May 16, 2025, at the intersection of La Cienega and Jefferson Boulevard in Los Angeles, Ms. Haven was operating the subject vehicle when it was involved in a hit-and-run that resulted in a secondary collision with multiple vehicles. Photos (attached) document the significant impact. Emergency responders were required to extricate Ms. Haven from the vehicle, during which they informed her that the airbags failed to deploya”a fact also documented in the official LAPD report. The failure of the airbag system represents a significant and dangerous malfunction of the vehicleas safety features. This is particularly troubling in light of Toyotaas recent recall, which affects more than 400,000 vehicles.
